Conduct psychosocial assessments, crisis interventions, evaluations, education, and care management of patients. Focus on behavioral health treatment in time-limited therapy for a limited number of patients; assist with referrals for higher levels of care. Collaborate with medical and behavioral health providers on patients' treatment goals. Develop behavioral change, educational, and support groups for patients to address high-risk behaviors and health improvement strategies. Participate in regular all-staff, departmental, and educational meetings, representing behavioral health perspectives. Fulfill administrative and documentation responsibilities, including electronic medical records and managed care authorizations. Perform all other duties as assigned and/or required. About Brookside Community Health Center: Brookside is a licensed Community Health Center of Brigham and Women's Hospital, providing multi-disciplinary, comprehensive care in various medical areas including Adult, Family, and Pediatric Primary Care, OB/GYN, Family Planning, Dental/Oral Surgery, Nutrition/WIC, Behavioral Health, and Specialty Medicine. Each year, more than 12,000 patients from Jamaica Plain and surrounding areas utilize these services. Qualifications: Education: Master of Social Work Degree (MSW) from an accredited program required. Licensure: Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW), or must obtain MA LCSW within 4 months of hire date. Language: Native or near-native fluency in Spanish required. For LCSW hires, obtaining the MA LCSW licensure within four months of the start date is a condition of continued employment. Skills/Abilities/Competencies Required: Demonstrated flexibility, autonomy, and decision-making capacity. Extensive knowledge of current social work theory and practice. Good clinical skills in assessment, crisis intervention, and evidence-based behavioral health treatment. Ability to make appropriate decisions under pressure and meet established productivity standards. Sensitivity to diverse cultures, beliefs, and lifestyles. Excellent patient interaction and staff collaboration skills. Strong communication skills, both orally and in writing.
