Patient Navigator Temporary Employment Program Temporary Employment 82191BR Job Summary The duration of this temporary role is approximately six months. Will include optional medical coverage, sick leave accrual, holiday pay and vacation accrual. OBIC is in the Division of Substance Use And Addiction Medicine (DSAAM), which is a part of the UCSF Department of Psychiatry and Behavioral Sciences (DPBS) at Zuckerberg San Francisco General Hospital (ZSFG). DSAAM is dedicated to providing culturally competent services to the diverse patient population we serve. Providing a range of clinical services informed by evidence-based practices, cultural humility, and trauma-informed care. DSAAM consists of a number of programs located at Zuckerberg San Francisco General Hospital and Trauma Center and within the community including OBIC Clinic, Opioid Treatment Outpatient Program (OTOP), Methadone Van, Office-based Opioid Treatment Program, and Stimulant Treatment Outpatient Program. OBIC (Office-based Buprenorphine Induction Clinic) is operated through the UCSF/ZSFG Department of Psychiatry and Behavioral Sciences in partnership with the San Francisco Department of Public Health (SFDPH). OBIC provides evidence-based outpatient treatment for substance use disorders with particular emphasis on medications for addiction treatment. OBIC is starting a new patient outreach and engagement program. We are seeking a Patient Navigator to play an essential role in outreach and engagement to people who are using drugs and would like to seek care. This is an exciting time to join our team as we grow and transform with the addition of new programs and services to support patients and our community. We are looking for a mission-driven, team player committed to patient-centered and compassionate care for people using drugs. The patient navigator will work closely with the OBIC team to engage and outreach to patients in care focused on low threshold access to medications and harm reduction. The patient navigator will play an essential role in supporting patients and collaborating with the OBIC team. The ideal candidate will utilize lived experience with substance use and/or living in the communities we serve to support patients engaging in care. We aim to foster an environment of inclusion and belonging for our workforce and to create a team that reflects the diversity of San Franciscans served in our clinic. The Patient Navigator will provide support to individuals coming to or who have been referred to OBIC clinic. The patient navigator will conduct outreach to patients who have been referred to OBIC, facilitate appointment barriers, serve as a resource and coach for their clients, and identify and address barriers to care. They also assist with access to other services such as health insurance, primary care, mental health services, social services, and residential treatment facilities. The patient navigator will also work closely with the OBIC Substance Use Counselor to serve as a resource for patients, develop treatment plans, and assist with documentation. The navigator will utilize a strengths-based approach and may utilize applicable lived experience and other skills to establish trust, ensure safety and reduce trauma that clients experience. The Patient Navigator serves as a non-clinical resource and liaison for patients and their families. Assists with navigating all aspects of the healthcare system. Uses knowledge of the unit or clinic to resolve issues and identify ways to increase patient satisfaction. Collaborates with department managers to meet patient expectations, achieve quality outcomes and build a patient focused culture. Preference for applicants with connections to, and reflecting the diversity of, the local community. Key responsibilities: Engagement and Outreach: Outreach to new referrals to connect and re-engage with patients. Act as a liaison with community treatment providers and referral sources as needed. Make contact information widely available to current and prospective patients. Establish a relationship with patients and communicate via telephone, texting, and/or email. Remind patients of appointments, help navigate obstacles to follow-up treatment, and provide encouragement. Work with existing patients by identifying barriers to care. Use Motivational Interviewing techniques to communicate in a respectful, culturally appropriate, and non-judgmental manner. Promote harm-reduction strategies and treatment goals based on patients' goals, preferences, and values. Provide culturally competent counseling services. Follow Wellness&Trauma-Informed principles and utilize lived experience to establish peer support. Work with patients to establish treatment plans and goals. Address patient concerns or barriers and identify appropriate resources as needed. Care Navigation: Assist with navigating barriers to patients obtaining medications from the pharmacy. Develop connections with a comprehensive array of community service providers to address the needs of people with SUD or co-occurring mental health disorders. Assist patients with connection to Medi-Cal enrollment, primary care, mental health services, social services, and residential treatment facilities. Assist out-of-county patients or those with private insurance in accessing treatment and other services in the appropriate system. Administrative: Submit appropriate paperwork and documentation within the electronic health record (Epic and Avatar) and data capture systems related to programmatic services and participant contacts. Adhere to all State, Federal, and County documentation standards and HIPAA regulations. Attend required trainings and regular team meetings. Collaborate with managers and colleagues around program tracking and improvement. Perform point of care urine drug screens as needed. Demonstrate commitment to racial equity and continued development of multi-cultural understanding and professional development through trainings, workshops, readings, etc. Assist in program operations. Strong aptitude to provide culturally and linguistically appropriate behavioral health services to the population of focus. Ability to remain calm under pressure and apply sound judgment. Strong skills to work as part of a team, to prioritize and handle multiple tasks simultaneously and work independently in a fast-paced environment with minimal supervision. Ability to work collaboratively with other community agencies. Strong ability to uphold professional boundaries Demonstrated excellent attendance and reliability. Bachelor's degree in related area and / or equivalent experience / training. Preferred Qualifications Ability to speak, write, and understand Spanish. Social Work experience. Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.
