Alaris Health at Kearn, a 120 bed-skilled nursing Center in Kearny, New Jersey, seeks a social worker to plan and implement social service programs in the Center and coordinate discharge planning. Additional responsibilities include: Planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families. Evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan. Coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families/responsible parties, and the treatment team. Maintaining residents’ rights at all times while liaising between all parties involved. Coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents. Ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights. JOB REQUIREMENTS: Bachelor’s degree or higher in social work or human services (knowledge of family assessment and interventions is preferred). Certifications required by New Jersey state regulations. Excellent command of the English language (written, read, and spoken), excellent interpersonal skills, and knowledge of all governmental regulations relating to the social services department. Ability to work independently, problem solve, and make decisions as necessary. Ability to positively interact with everyone from residents to government agencies and the general public. J-18808-Ljbffr
