What's in it for you: Growth opportunities to uplevel your career A people-centric culture embracing and celebrating diverse perspectives, backgrounds, and experiences Competitive compensation and comprehensive benefits focused on well-being An opportunity to shape the future of health care by joining a team recognized as a Best Place to Work in New York Qualifications you'll bring: Licensed Social Worker in NYS, with demonstrated clinical experience and previous experience working with the Medicaid population At least two years of case management experience required in long-term services support Must have a NY state ID or driver's license Utilization review experience and experience completing authorizations in long-term support services preferred Ability to solve problems while exhibiting clinical judgment and understanding of the current environment Experience in situations demanding strict adherence to confidentiality Ability to make independent decisions about appropriateness, resource utilization, and quality of care Understanding of clinical and psychosocial issues affecting treatment or care plans Knowledge of member-centric, motivational interviewing strategies, and strength-based models a plus Curiosity to foster innovation and pave the way for growth Humility to play as a team Commitment to being the difference for our customers in every interaction Your key responsibilities: Provide MVP members enrolled in the Medicaid Long Term Support Program with psychosocial support needed to cope with chronic, acute, or disabling conditions Telephonically assess, identify needs, coordinate, educate, case manage, and/or coach members Provide member education and counseling, make necessary referrals to social service programs, and coordinate necessary services for MVP members in the Medicaid LTSS program Establish and maintain relationships with community agencies to provide contracted and alternative services when appropriate Refer members and/or families to community resources to assist in recovery from physical or long-term illness Participate in discharge planning and care coordination meetings with multiple providers Collaborate with other professionals to evaluate patients' medical or physical condition and assess member needs Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan Follow case management cases to successful completion using appropriate documentation Contribute to our pursuit of excellence by performing various responsibilities that may arise Where you'll be: Virtual, within New York State.
