Job Description&Requirements RN Case Manager StartDate: 12/9/2024 Pay Rate: $2000.00 - $2800.00 POSITION SUMMARY RN Case Manager - UM POSITION DUTIES provide utilization management of patients in SNF, conducts SNF authorization for billing, inputs the billing referrals in EPIC EMR, conducts discharge planning on skilled patients, supports physicians managing the SNF patients, manages quality / care experience of the patients.
