Job Title: RN - CASE MANAGEMENT Contract Duration: 13 weeks Shift: 7:30 AM - 4:00 PM (Friday weekend rotation) Location: El Paso, TX Job Description: We are seeking a highly experienced and assertive RN Case Manager for a 13-week contract assignment. The role involves managing patient care from admission to discharge, with a focus on progression of care and discharge planning. This position requires collaboration with a multidisciplinary team and families, often involving difficult discussions. Key Responsibilities: Perform case management duties including progression of care, discharge planning, and multidisciplinary rounds. Utilize Meditech and MIDAS systems for patient management and documentation. Communicate effectively with families, physicians, and the care team regarding discharge plans.
