Job Summary: The Providence Center Residential Case Manager Ledge Street Supported Apartments will provide clinical case management services to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address mental health symptoms and co-occurring disorders. Work with clients to achieve the highest level of independence possible. Provide psychosocial rehabilitation services to facilitate program goals and objectives. Schedule: Friday 11pm-7am, Saturday 11pm-7am Duties and Responsibilities: Provide case management and support to consumers. Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment. Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments. Initiate treatment and advocate for consumers within the network of care. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, and consumers themselves. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. Transport consumers in own vehicle or Center van to planned activities and appointments. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. Maintain cooperative relationships with Center staff, consumers, community agencies, and the public. Attend trainings, case presentations, and conferences as required. Other duties may be assigned. Work flexible hours, including evenings, weekends, holidays, and during inclement weather. Requirements: High School Diploma/GED required. Experience in residential facilities is helpful. Must have valid driver's license and insured auto. Ability to read and interpret instruction documents and follow procedures. Ability to write routine reports and correspondence. Ability to speak effectively with consumers, employees, and individuals in the community. Must be able to work flexible hours including evenings, overnights, weekends, and holidays as well as cover the program when unforeseen circumstances arise to ensure 24-hour coverage. J-18808-Ljbffr
