Overview We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in various therapeutic techniques, including orthopedics and trigger point therapy, and will be committed to providing high-quality patient care across multiple settings. This role involves working with patients in inpatient, outpatient, and home health environments, focusing on rehabilitation and recovery to enhance their quality of life. Responsibilities Conduct comprehensive assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on patient evaluations and goals. Implement therapeutic exercises and interventions tailored to each patient's needs. Utilize knowledge of medical terminology to effectively communicate with healthcare teams. Provide education to patients and their families regarding treatment plans, discharge planning, and home community care strategies. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Collaborate with other healthcare professionals in acute care settings to deliver integrated patient care. Maintain accurate documentation of patient treatments, progress notes, and discharge summaries in accordance with facility policies. Experience A degree in Physical Therapy from an accredited institution is required. Previous experience in orthopedics, home health, or outpatient settings is preferred. Familiarity with trigger point therapy techniques is a plus. Knowledge of discharge planning processes and systems used in various care environments is essential. Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams are necessary for success in this role.
