Job Overview We are seeking a dedicated and compassionate Physical Therapist to join our team. In this role, you will be responsible for evaluating and treating patients with various physical conditions, helping them to improve their mobility and quality of life. You will work closely with patients of all ages, including pediatrics and those in acute care settings, providing personalized treatment plans tailored to their specific needs. Responsibilities Conduct thorough assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on patient evaluations and goals. Implement therapeutic interventions to enhance patients' mobility and functional abilities. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families on exercises, techniques, and home health care strategies. Collaborate with other healthcare professionals to ensure comprehensive patient care. Participate in discharge planning to facilitate a smooth transition for patients from inpatient care to home health or outpatient services. Skills Strong knowledge of anatomy and physiology related to physical therapy practices. Experience in patient care, particularly in home health environments. Proficiency in discharge planning processes for effective patient transitions. Familiarity with pediatric physical therapy techniques is a plus. Ability to work effectively in acute care settings, providing timely interventions as needed. Excellent communication skills for educating patients and collaborating with healthcare teams. Strong organizational skills to manage multiple patient cases efficiently.
