Provide hands-on physical therapy treatments, including exercises, manual therapy, and modalities. Collaborate with healthcare professionals to coordinate discharge planning and ensure continuity of care. Educate patients and their families about treatment options, exercises, and preventive measures. Maintain accurate documentation of patient progress and treatment outcomes. Stay updated on the latest physical therapy practices and advancements in the field. Skills Strong knowledge of anatomy and medical terminology relevant to physical therapy. Experience in home community care and home health settings is preferred. Proficiency in discharge planning processes to facilitate smooth transitions for patients. Ability to create effective care plans tailored to individual patient needs. Familiarity with orthopedic conditions and treatments.
