Overview We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will provide high-quality patient care in both outpatient and acute care settings, focusing on developing and implementing effective treatment plans tailored to each individual's needs. This role requires a strong understanding of physical therapy practices, particularly in geriatrics and pediatrics, as well as experience in discharge planning and care plan development. Responsibilities Conduct thorough evaluations of patients to assess their physical condition and needs. Develop individualized care plans that align with patients' goals and rehabilitation requirements. Implement treatment protocols using various physical therapy techniques and modalities. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Collaborate with other healthcare professionals to coordinate comprehensive patient care. Educate patients and their families on exercises, techniques, and strategies for recovery. Maintain accurate documentation of patient evaluations, treatments, and progress notes. Participate in discharge planning to ensure a smooth transition for patients from therapy to home or other care settings. Qualifications Doctor of Physical Therapy (DPT) degree from an accredited program. Valid state license to practice physical therapy. Experience in outpatient settings preferred; familiarity with geriatrics and pediatrics is a plus. Strong knowledge of patient care practices, including acute care environments. Excellent communication skills for effective interaction with patients, families, and healthcare teams. Ability to develop comprehensive care plans that address the unique needs of each patient.
