Type: Full-time, Part-time, or PRN (Home Care and Senior Living Facilities)
Position Overview:
We are seeking a Physical Therapist to join our growing team in the Madison, AL area. This position focuses on providing outpatient therapy within a mix of home-based settings and Senior Living Communities/Independent Living Facilities. The role is flexible, allowing for full-time, part-time, or PRN work, with a minimum requirement of 10 hours per week.

• Availability: Regular weekly availability required, including 3 weekdays
• Population: Geriatric
• Care Setting: A mix of house calls and care in Senior Living Communities (SLC) and Independent Living Facilities (ILF), billed as Medicare Part B
Job Responsibilities:
• Provide community-based outpatient services (Medicare Part B) in the homes of older adults or within Senior Living Communities.
• Deliver 1-on-1 direct patient care to a well-balanced caseload of geriatric patients.
• Ensure regular weekly availability, with flexible hours based on your schedule preferences.

Job Requirements:
• Doctor of Physical Therapy (or equivalent) degree
• Eligibility for Alabama state licensure as a Physical Therapist
• Availability to work a minimum of 10 hours per week
• Regular and consistent weekly availability, including 3 weekdays
• Professionalism, accuracy, and dependability are essential
• New Grad PTs will be supported and mentored
• For PRN candidates, prior work experience is required
