*Responsibilities:*
* Travel home to home within your coverage area and provide physical therapy services to patients following their individual care plans.
* Conduct assessments and develop treatment plans based on patients' needs and goals.
* Educate patients and their families on exercises and techniques for at-home care.
* Document patient progress and maintain accurate records in EMR systems.
* Collaborate with healthcare team members to ensure comprehensive patient care.
* Stay updated on the latest research and advancements in physical therapy practices.

*Requirements:*
* Current Physical Therapist PT License
* CPR Certification (AHA or Red Cross)
* 1 Year Healthcare Experience
* Physical Resilience
* Positive Attitude
* Dedication to Quality Care
* Strong Communication Skills
