Our Values: Be Kind, Build Trust, Innovate, Go the Extra Mile (Be BIG) COULD THIS BE YOU As a Clinical Pharmacist, PGY2 Population Health Resident , you will provide patient-centered care to optimize medication therapy to improve clinical outcomes, reduce the overall cost of care, and increase patient and provider satisfaction. The clinical pharmacist performs medication management services to identify, resolve, and prevent medication-related problems. As the Clinical Pharmacist, PGY2 Population Health Resident, you will also support the pharmacy department in driving clinical pharmacy performance across all value-based care contracts. You will be a member of an enterprise-wide central clinical pharmacy team providing clinical services, reporting, performance benchmarking, process improvement, best practice sharing, training, and education. EXAMPLES OF DUTIES Drive clinical outcomes, operational goals, practice engagement, and care management coordination in the assigned market(s). Develop and maintain clinical practice with a patient care service, cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provide patient-centered, evidence-based, pharmaceutical care through provider-delegated collaborative practice agreements, High-Risk Clinic, Village Medical Home, Care Management programs. Ensure patients have a positive experience by providing a high level of service promptly, professionally, and empathetically. Communicate professionally and discuss/document clinical recommendations/interventions to prescribers and interdisciplinary care teams. Follow up with patients and/or caregivers and appropriately document outcomes. Serve as a clinical resource and consultant in all pharmacy and medication management areas for care management and clinic teams. Establish professional relationships with key stakeholders, including medical directors, market operations, care management, population health, pharmacy integration, market engagement, technology, analytics, and clinical teams. Support the Population Health and Clinical Quality team on the implementation of best practices and workflows to support maximum data/analytics performance in quality, patient, and efficiency experience. Serve as the main point of contact on special projects as assigned in partnership with the Data/Analytics Director, Clinical Quality Director/Manager, and preceptor. Prepare, compile, review, and submit weekly, monthly, and quarterly reports for leadership teams. Participate in clinical guideline development, critical pathways, disease management, and drug use programs. Develop and deliver clinical education and training to students, pharmacists, and providers. Support in improving clinic NCQA, HEDIS, and CMS STAR quality measure. Complete residency research project. Meet productivity and performance expectations set forth by preceptors and ASHP accreditation. Perform all other duties as assigned to meet departmental objectives. EXPERIENCE TO DRIVE CHANGE Completion of a Pharmacy Postgraduate Residency (PGY-1) required. PharmD from an accredited college of pharmacy required. Active, unrestricted pharmacist license in the state of Texas required. Relocation to Houston, Texas required. Proven project management experience including program development and implementation and identifying and deploying cross-organization best practices preferred. This role requires travel up to 40%.
