North Mississippi Health Services is seeking a Social Work Social Worker for a per diem / prn job in Tupelo, Mississippi. Job Description&Requirements Specialty: Social Worker Discipline: Social Work Duration: Ongoing Employment Type: Per Diem Patient Care/Assessment: Assess social and emotional factors relating to/affecting health status of patient Assess/identify patient/family needs, situations, strengths, and support networks to determine their goals Meet with patient for routine follow up visits Care Planning: Identify and evaluate high-risk patient situations Identify needs and develop plans to improve the well-being of the patient Discuss options with patients/family members Research and refer clients to community resources (i.e. food stamps, child care, and health care) Assist patients with government agencies (i.e. apply for and receive Medicare benefits) Complete referrals as needed Liaison: Serves as a liaison between patient(s) and community resource providers Education: Educates patient/family members on services to help with the management of illnesses and/or diseases Participates in staff and/or community educational programs Reporting/Recordkeeping: Accurately and efficiently document patient treatment plans, goals, and intervention(s) per Departmental Guidelines Requirements: Master’s Degree in Social Work. Currently be a Licensed Medical Social Worker in the State of Mississippi required. Minimum of five years of experience in a health care setting as therapist required. Must be knowledgeable various therapy techniques. Must be knowledgeable of the principles of growth and development over the life span. Must work independently with minimal guidance and supervision.
