In addition, we are committed to prioritizing racial and economic justice and health equity in all our programs and business practices. We are a dynamic and fast-growing agency at the forefront of the "Food is Medicine" field, providing over 1M medically tailored meals annually to clients and their families experiencing illness and food insecurity in Massachusetts and Rhode Island. Our work strongly focuses on supporting the regional local food system, delivering high-quality nutrition education to the community, and hosting an on-site food-service workforce development program. Over the past 34 years, the agency has provided 13M free, scratch-made meals. Summary : Reporting to the Senior Director of Strategy and Business Development and in collaboration with other agency staff, the Outreach Specialist will perform outreach and develop relationships with referral partners, including but not limited to health centers and physician practices. This will involve the following: Manage and grow relationships between Community Servings and new and existing referral partners through regular meetings, communications initiatives, and other outreach activities. Working with agency Data Analyst, use internal and external data sources to identify opportunities for new relationship development and outreach. Work closely with members of the Program team to ensure coordinated handoff of referrals. Partner with agency Communications teams to develop outreach collateral and processes for assisting referral partners and clients to easily access our services. This is a hybrid position, requiring in-person work at our offices in Jamaica Plain and Mansfield, as well as meetings, presentations, and visits with external referral partners, with the ability to work remotely. Essential Duties and Responsibilities include the following. Other duties may be assigned. Meet with new and existing referral partners, including office managers, physicians, case managers, social workers, and others to build awareness, strengthen relationships, grow referral volume, and triage feedback. Develop and cultivate a list of referral partners based on Community Servings referral data and external sources, focusing on populations that serve pregnant individuals, the pediatric population, and people living with HIV/AIDS. Lead regularly scheduled meetings (in person and virtual) with new and existing partners – developing agenda, preparing collateral and reports, with an effort to grow client referrals. Attend community outreach events to promote Community Servings’ nutrition programming. Collaborate with the agency Program team to develop a process for ensuring easy access for referral partners and clients to our Medically Tailored Meals (MTM) program. Monitor trends in referrals and track incoming referrals. Triage any referral submission issues and keep up to date on programmatic changes. Establish and maintain an outreach schedule for themselves as well as Community Servings content experts in the community. Provide background information to Community Servings staff in advance of external speaking engagements. Collaborate with Community Servings Culinary team to develop a plan for referral partner lunches. Collaborate with Grants team to meet population-specific grant requirements. In collaboration with the Communications team, implement communication strategies designed to promote Community Servings awareness in the healthcare community. Identify opportunities for Community Servings clinical staff to present at Continuing Medical Education (CME) events within the referrer community. Maintain and synthesize feedback from referral partners for review by agency Program and Leadership team. Work with Data Analyst, Salesforce Administrator, and other teams across the agency to implement referring provider database and other tracking tools to measure the effectiveness of the program. Maintain a collaborative, team relationship with peers and colleagues to effectively contribute to the working group’s achievement of goals. Bachelor’s Degree preferred, but not necessary. Demonstrated sales and/or project management experience. Self-starter with well-developed interpersonal, communication, and presentation skills. Demonstrated organizational, time-management, and project management skills. Ability to set priorities and manage multiple tasks and deadlines simultaneously. Ability to work independently. Ability to travel to off-site meetings throughout Massachusetts and Rhode Island.
