Area of Interest Providers Position Type Full-time Job Description We are currently seeking an experienced Nurse Practitioner to join our Burbank, CA and Glendale, CA clinical team. This individual will provide in-home health care, assessments and patient care in assigned territory. Essential Duties and Responsibilities include the following: Visit members with complex care needs at home (5-7 members per day) Perform comprehensive health assessment which includes but is not limited to the following: Obtain history and perform physical examination Complete review of medicines and related instructions Review home safety and psychosocial needs Fulfill documentation needs for STAR and HCC measures Obtain and document all pertinent information in patient's charts and case management notes on the same day as the patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life-threatening emergencies Maintain patient safety and confidentiality Universal precautions utilized in all patient care Prescribe/order procedures, treatments and medications within scope of practice Rotate with the inpatient hospitalist's team for an assigned frequency and duration to be determined by the Campus Medical Director in order to maintain excellent clinical skills Evaluate patients at the Post Discharge Clinic with the attending Physician Develop high level of trust and intimacy with patients and families All other duties as directed by management. The pay range for this position at commencement of employment is expected to be between $135,000 - $140,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. If hired, the employee will be in an “at-will position” and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors. Requirements Required Experience: At least 1 year of prior experience Home health experience is necessary Education/Certification Requirements: Graduation from an accredited Nurse Practitioner program J-18808-Ljbffr
