We are offering a healthy work-life balance with flexibility, great benefits AND MORE We are seeking a compassionate and experienced Nurse Practitioner (NP) to join our skilled nursing facility team. The ideal Nurse Practitioner (NP) will have a valid Nurse Practitioner license and 1-2 years of experience in a clinical setting, preferably in long-term care, geriatrics, or skilled nursing. As a Nurse Practitioner (NP) in our facility, you will play a key role in delivering high-quality care to our residents, collaborating with physicians, nursing staff, and other healthcare professionals to ensure optimal patient outcomes. Benefits: • Paid time off • Health Insurance • Malpractice Insurance • 401k Job Qualifications: Education Licensure: • Active Nurse Practitioner license in NJ. • Certification in Adult-Gerontology Acute Care Nurse Practitioner (AG-ACNP) or Family Nurse Practitioner (FNP) preferred. Experience: • Minimum of 1-2 years of clinical experience as a Nurse Practitioner, preferably in skilled nursing, long-term care, geriatrics, or a similar healthcare setting. • Experience with geriatric populations and complex medical conditions highly desirable. Skills Competencies: • Strong clinical assessment and diagnostic skills. • Ability to work independently as well as collaboratively within a multidisciplinary team. • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and staff. • Knowledge of long-term care regulations, clinical practices, and documentation standards. • Familiarity with electronic medical record (EMR) systems. Responsibilities include but are not limited to: • Clinical Care: • Provide comprehensive healthcare services to residents, including physical assessments, diagnostic evaluations, and treatment planning. • Diagnose and manage acute and chronic medical conditions commonly found in the elderly population, including but not limited to cardiovascular, respiratory, gastrointestinal, and infectious diseases. • Order and interpret diagnostic tests (e.g., lab work, radiology). • Prescribe medications and therapies within the scope of practice and in accordance with facility protocols. • Monitor residents' progress and adjust treatment plans as necessary. • Perform physical exams and health assessments, documenting findings accurately. • Collaboration and Communication: • Work closely with the medical director, attending physicians, nursing staff, and interdisciplinary team to provide integrated care. • Provide patient and family education on treatment options, disease prevention, and health promotion. • Communicate effectively with residents and families regarding care plans, changes in condition, and other important healthcare information. • Participate in care conferences and contribute to the development of individualized care plans. • Documentation and Compliance: • Maintain accurate, timely, and complete medical records in compliance with facility policies and regulatory standards. • Ensure all clinical documentation is in accordance with state and federal regulations. • Complete required reports, forms, and assessments for residents ongoing care needs. • Quality Improvement Education: • Participate in quality improvement initiatives to enhance patient care and outcomes. • Stay current with developments in geriatric care, clinical guidelines, and best practices. • Educate and mentor nursing staff, providing guidance on clinical procedures, medications, and patient care. • Emergency Response: • Provide immediate care in emergency situations and collaborate with emergency medical services if required. • Assess and triage patients presenting with urgent or emergent health concerns.
