Position: LPN Specialty: Long-Term Care. Location: Denver, Colorado. Duration: 13 weeks with possible extension Shift: 12 hours, Day 6:00 AM-6:00 PM Guaranteed Hours: 36 hours in a week Responsibilities: A Licensed Practical Nurse (LPN) working in a Long-Term Care (LTC) facility has specialized responsibilities focused on providing care to elderly or chronically ill patients who require ongoing assistance with daily living and healthcare needs. Below are the typical responsibilities of an LPN in a Long-Term Care (LTC) setting: 1. Patient Care and Monitoring Monitor Health Status: Regularly check vital signs such as blood pressure, temperature, heart rate, and respiration, and report any abnormalities to senior healthcare providers. Assist with Activities of Daily Living (ADLs): Help residents with bathing, dressing, grooming, eating, and toileting. Manage Chronic Conditions: Assist in managing long-term conditions like diabetes, heart disease, and respiratory illnesses, following care plans and physician orders. Provide Basic Wound Care: Assist with dressing changes, observe for signs of infection, and report any issues to the supervising nurse or doctor. 2. Medication Administration and Management Administer Medications: Administer prescribed oral, topical, or injectable medications, and ensure that medications are given at the correct time and in the right dosage. Monitor Medication Effects: Observe residents for any side effects or adverse reactions to medications and report to the physician or RN. Medication Documentation: Accurately document all medications administered and any changes in patient condition related to medication. 3. Care Coordination Follow Care Plans: Implement individualized care plans developed by the care team, including RNs and physicians, ensuring all treatment needs are met. Assist in Care Planning: Provide input during care plan meetings to assess and update care needs based on the resident’s condition and changes in health. Collaborate with Care Team: Work closely with RNs, physicians, physical therapists, and other healthcare professionals to ensure comprehensive care for each resident. 4. Resident and Family Education Educate on Health Maintenance: Teach residents and their families about managing chronic conditions, medication, nutrition, and other health-related topics. Provide Emotional Support: Offer comfort and reassurance to residents and families, addressing concerns and helping manage emotional or mental health challenges. 5. Documentation and Reporting Chart Patient Information: Maintain accurate and up-to-date records of resident health status, treatments, medications, and any changes in condition. Report Significant Changes: Communicate changes in resident condition to the charge nurse, physician, or other healthcare team members. Ensure Compliance: Adhere to LTC facility policies and state regulations, ensuring all required documentation is completed in a timely and accurate manner. 6. Emergency Response Handle Emergencies: In case of an emergency, respond quickly by performing first aid, CPR, or other interventions as needed until further medical help arrives. Assist with Evacuation or Safety Protocols: Ensure residents are safe during drills or actual emergency situations, following facility protocols. 7. Nutritional Support Monitor Food Intake: Assist residents with meals, particularly those who need help eating, and monitor for signs of malnutrition or dehydration. Ensure Dietary Needs are Met: Follow specific dietary guidelines prescribed by the healthcare provider, such as low-sodium, diabetic, or gluten-free diets. 8. Behavioral Support Manage Behavioral Symptoms: Work with residents who may have dementia, Alzheimer's, or other cognitive impairments, addressing issues like agitation, confusion, or aggression. Maintain a Safe Environment: Ensure a safe and calm environment for residents, minimizing the risk of falls or accidents, particularly for those with mobility challenges. 9. Supervision and Delegation Supervise Nursing Assistants: Oversee Certified Nursing Assistants (CNAs) or other nursing staff, ensuring they perform their duties properly and provide quality care to residents. Delegate Tasks: Assign appropriate tasks to CNAs and other support staff, ensuring that resident needs are met promptly and efficiently. 10. Administrative and Facility Support Inventory Management: Help maintain adequate supplies of medications, medical equipment, and other materials needed for patient care. Assist with Admissions and Discharges: Help admit new residents or discharge patients when they are transferred to another facility or discharged home, ensuring smooth transitions. LPNs in LTC facilities play a crucial role in providing hands-on care to residents, ensuring their physical, emotional, and social well-being. Their responsibilities are central to maintaining quality of life and comfort for long-term residents who often have complex and ongoing healthcare needs.
