Job Summary We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients, ensuring their comfort and well-being while adhering to established standards of practice. This role involves working closely with patients of all ages, including children. Key Tasks and Responsibilities Assists physicians as part of the clinical care team Prepares patients for examination and treatment by physicians and mid-level providers by taking vital signs and taking histories Prepares and maintains clinical equipment Triages and responds to patient communication following established protocols. Provides education to patients on various diseases and treatment strategies in accordance with orders. Collaborates with case managers, behavioral health and other staff to assure that follow-up care is received. Understands UDS/Quality Improvement data measures and goals. Assures that needed medical supplies are available when needed. Assures that clinical areas are clean, safe and that processes assure the safety of patients from cross contamination. Maintains strict confidentiality and understands HIPAA requirements Charts relevant patient information in an electronic health record. Assist patients in accessing lab testing, medications, specialty care and social services necessary to promote a healthy lifestyle Maintains sample medication records in accordance with accreditation requirements. Treats patients, family members, and other staff with dignity and respect. Skills and Attributes Possesses knowledge of outpatient medical practice Knowledgeable of medical equipment and instruments Maintains strong working relationship with other staff and departments in the Center. Has a teamwork orientation to promote the provision of holistic health services. Knowledge of common safety, infection control and hazard communication. Has an understanding of community health centers and the Uniform Data System requirements. Possesses basic arithmetic skills Is comfortable treating patients with complex needs including behavioral health, poverty-related issues and disability issues. Possesses a solid working knowledge of community health resources Physical Demands The work performed is in an outpatient clinic setting. Duties may require long periods of standing, bending, reaching, carrying, walking and lifting up to 30 pounds. Must be physically ability to support patients in who may need some physical assistance with standing, sitting, etc. Qualifications Licensed Practical Nurse, Registered Nurse and/or Certificate of completion for an MA (LPN/RN/MA) in Missouri. Must possess a certificate from an PN/Nursing, Medical Assistant program or diploma from a accredited school of nursing. Pay commensurate on experience.
