*Responsibilities:*
_As a Hospice Registered Nurse Case Manager, you will:_
* Provide high-quality care to patients in the comfort of their homes, playing a crucial role in their healthcare journey.
* Collaborate with a multidisciplinary team to ensure the delivery of timely and appropriate care.
* Foster meaningful connections with patients and their families, offering support and compassion during the end-of-life phase.
* Develop and implement individualized care plans based on patient assessments and goals.
* Conduct regular visits to patients, providing symptom management, pain control, and emotional support.
* Communicate effectively with patients, families, caregivers, and other healthcare professionals to ensure coordinated care.
* Document patient care activities accurately and timely in accordance with regulatory standards and organizational policies.
* Participate in interdisciplinary team meetings to review patient progress, discuss care plans, and address any challenges.
* Provide education and support to patients and families regarding end-of-life care, advance directives, and bereavement support.
*Requirements:*
_To qualify for this position, candidates must have:_
* A positive attitude and dedication to providing quality patient care.
* Strong communication skills to effectively interact with patients, families, and colleagues.
* Reliable transportation to fulfill job duties.
* An Advantage Program Certificate, where applicable by state regulations.
* A Bachelor's degree, along with one year of additional paid professional experience or one year as a Registered Nurse.
