They operate 4 clinic locations, with a robust support staff of clinical and administrative team members. They are currently hiring due to growth. POSITION DESCRIPTION: The new FNP will provide comprehensive medical services for patients of all ages. FNPs see between 16 and 24 patients per day, with the support of a dedicated clinical assistant. Specific duties of the role include, but are not limited to: evaluation and treatment of patients care plan management assessment of acute needs chronic condition management review laboratory and other test results initiate specialist referrals participate in inter-departmental case conferences participate in care team, provider, and staff meetings provide health education to patients and families participate in community outreach events The practice uses the Epic EHR.
