We are searching for compassionate, dynamic, energetic, organized, and efficient professionals with home health and hospice experience to become part of our family servicing clients in the Petoskey and surrounding areas In this role, you will be responsible for providing skilled nursing care to patients in their homes and offering emotional support to patients and their families. You will conduct supervisory visits to observe the performance of assigned team members and participate in interdisciplinary group meetings to develop and revise the patient/family plan of care.
