Job Description

Join one of New York's largest community health centers with many decades in healthcare. The FNP oversees the clinical operations of the assigned practice and programs, ensuring compliance with organizational goals and regulatory standards.

Job Function:

Conduct comprehensive health assessments and diagnose acute and chronic illnesses.
Develop and implement patient treatment plans and monitor outcomes.

Prescribe medications and other treatments as necessary, in accordance with state regulations and practice guidelines.

Perform procedures such as suturing, wound care, and joint injections.

Educate patients and families on preventive care, wellness, and health maintenance.

Collaborate with physicians and other healthcare professionals to coordinate patient care.

Maintain accurate and up-to-date patient medical records using electronic health records (EHR) systems.

Participate in quality improvement initiatives and patient care conferences.

Job Requirements:

Clinical Leadership experience preferred
NY State License
Board Certified Family Medicine
Current BCLS
Current DEA Certificate
