The practitioner will manage a panel of low income, culturally and linguistically diverse patients from newborns through seniors using an integrated, team-based approach to care by evaluating and treating patients, writing prescriptions, providing preventive health care advice and case management, assessing the psychological impact of illness, and treating or appropriately referring patients for additional evaluation and treatment. The state-of-the-art facility is affiliated with 3 Harvard Medical School teaching hospitals, uses an Electronic Health Record, and is working toward achieving Meaningful Use.
