The Emergency Room Enhancement (ERE) Care Coordinator coordinates access to primary care, behavioral health or other medical care to reduce the reliance on emergency room and inpatient hospital services. The Care Coordinator will possess a bachelor's degree in social work, psychology, mental health, human services or similar degrees. Nurses are also encouraged to apply. A master's degree (Qualified Mental Health Professional) is preferred. The Care Coordinator will work with uninsured, underinsured residents in Audrain, Callaway, Montgomery, Monroe, Pike and Ralls counties who frequently interact with law enforcment, urgent cares, hospitals, and ambulance districts. They will work to engage and support them in accessing the most clinically appropriate services and follow-up to assure that they have been connected with those services. The Care Coordinator will also be involved in collecting a reporting data on their activities. This position requires travel in the six-county area.
