Come work at the best place to give and receive care Job Description: Who We Are: Substance Use Disorder Services staff at The Elliot assesses, treats, and supports individuals with SUD who are in or seeking recovery. Our multidisciplinary team includes licensed mental health and substance use clinicians, psychiatric providers, RNs, case managers, and recovery support staff. Our team also works closely with primary care providers to ensure continuity of care. About the Job: The primary purpose of the Advance Practice Registered Nurse Practitioner is the provision of compassionate, effective, efficient, quality health, nursing, and medical care directly to patients. APRN’s practice autonomously in independent roles or in collaboration with other medical professionals across a variety of settings and provide the complete range of services consistent with standards established by a national credentialing or certifying body recognized by the National Council of State Boards of Nursing and approved by the New Hampshire State Board of Nursing in the appropriate APRN role and specialty in accordance with the State of New Hampshire Nurse Practice Act, Section RSA 326-B:11 (Scope of APRN Practice and Authority). What You’ll Do: Provides Assessment, treatment, and education of adult patients in the MOUD Treatment Program in accordance with a psychiatric specialty. Identifies psychological and substance use issues and needs based upon the assessment and evaluations and available data. Formulates differential diagnosis based on history, physical and clinical findings. Develops a treatment plan, in coordination with treatment program staff, which may include therapy, case management, outreach, pharmaceutical interventions as well as education. Follows progress of patient care to determine the effectiveness of the treatment plan and updates and records changes in health status and modifies treatment to achieve clinical goals as necessary. Based on the evaluation and treatment progress, prescribes medication, and identifies the need for multidisciplinary resources. Is accountable for recognizing the limits of his/her knowledge and experience and for planning the management of situations beyond APRN expertise by consulting with or referring patients to other health providers as appropriate. Promotes professional growth through mentoring of new staff or students as well as for interpreting the role of the nurse practitioner to individuals, families, and other professionals. Demonstrates consistently a high ethical standard and abides by all current legal standards as advocates for their patient(s). APRN’s promote advocacy activities that support their patients at the practice, hospital, community, state, or national level. Strives continuously to improve the quality of his/her patient care by developing, participating, disseminating, or interpreting current clinical research evidence and quality improvement data into practice at the local, community, state, or national level. Who You Are: Education: Must hold a graduate degree earned in an accredited advanced practice registered nurse education program or have graduated before July 1, 2004, from an APRN education program accredited by a national accrediting body. Licensure/certification: Must be currently certified by national certifying body in the specialty for which the applicant was educated that is recognized by the New Hampshire State Board of Nursing. Currently hold and maintain an RN and APRN license to practice in the state of New Hampshire or be eligible to obtain such license prior to date of hire. Experience: Shall possess at least 2 years’ experience as a registered nurse or advanced practice nurse; Additional education and experience may be necessary according to the specified role intended for the APRN practice area and may be added in an addendum.
