The Community Support Specialist works under the guidance of the Director of Supportive Living with a focus on health and safety management in an individual's home and implementation of individual service goals. As a Community Support Specialist, you will support people to live more effectively and meaningful lives by helping them secure nontraditional services that encompass a combination of staff support. This includes residential, employment, and community support. Key areas include assessing the needs, wants, and goals for all aspects of life for the persons supported, monitoring and advocating for services accordingly for individuals supported with disabilities and special needs, and connecting individuals to needed services. Provide intervention, assistance, and support for hospitalizations, injuries, death, and abuse, critical, and other emergencies deemed necessary. Prepare meals based on established menus, taking into account each individual's diet, health, and physical needs. Support individuals to ensure they are clean, groomed, and dressed appropriately. Responsible for daily activities of the facility as scheduled by individuals and staff, ensuring individuals are active in their communities. All other duties as assigned.
