*Job Description*
* Perform advanced clinical pharmacy duties to optimize pharmacotherapy for transplant patients.
* Conduct proactive and reactive medication regimen reviews to ensure safety and effectiveness.
* Collaborate with healthcare professionals to manage patient drug therapies and improve care transitions.
* Serve as a mentor and preceptor for pharmacy students and residents.
* Participate in quality improvement initiatives, including formulary management and medication use evaluations.
* Support innovative research and contribute to publications and presentations.
*Qualifications*
* *Education*: PharmD from an accredited institution.
* *Experience*:
* Completion of ASHP-accredited PGY1 residency (required).
* PGY2 residency in solid organ transplant or equivalent experience (preferred).
* *Licensure*: Eligible for Texas pharmacist licensure.
* *Skills*: Strong analytical and collaborative abilities, with a passion for patient-centered transplant care.
