Location: 1035 Cambridge Street Work Days: 9:00-5:00 pm Category: Human and Social Services Department: SAMHSA Safety Net Approach Job Type: Full time Work Shift: Day Hours/Week: 40.00 Union Name: Non Union Summary: The Children’s Health Initiative is looking for an experienced child and adolescent clinician to join our grant-funded interdisciplinary SafetyNet team at CHA. We are studying the cost effectiveness of intervening earlier to support youth and families struggling with barriers to accessing mental health care, particularly in situations involving childhood trauma, mental health disorders, and substance use. Our innovative approach combines pediatric integrated care delivery in primary care clinics, with key elements of systems-of-care, including:strength-based, multidisciplinary team assessments, individualized community-based care plans, and youth and family voice Responsibilities Able to function as a team leader, triaging needs and managing resources Works side by side with the Family Support Specialist - a peer to peer parent with lived experience parenting a child with mental health needs Able to communicate effectively with the consulting CHA child psychiatrist and with both internal (CHA) and external providers, schools and system partners Accessible onsite in primary care for “warm-handoffs” or curbside consults Key contact for PCPs; collaborates with primary care clinicians to prioritize new referrals and coordinate ongoing care Participates in shared “real time” child mental health assessment and follow-up visits with the child psychiatrist and the FSS Monitors implementation of recommended interventions by contacting the youth and parent to “check in” on evolving needs Helps the PCP “hold” the case while the family and youth get connected to sustainable resources Monitors implementation of recommended interventions by contacting the youth and parent to “check in” on evolving needs Helps the PCP “hold” the case while the family and youth get connected to sustainable resources Other Information Master's level clinician; LICSW preferred At least 3 years experience as a child and adolescent mental health clinician, post-training Familiar with both care delivery in outpatient medical settings, and in community based systems of care for children In keeping with federal, state and local laws, Cambridge Health Alliance (CHA) policy forbids employees and associates to discriminate against anyone based on race, religion, color, gender, age, marital status, national origin, sexual orientation, gender identity, veteran status, disability or any other characteristic protected by law.
