The BCM program plays a major role by linking clients with needed services in the community. A Blended Case Manager will advocate for his/her client; help negotiate complex systems such as special education and social services; assist in maintaining and finding access to basic living needs and skills such as employment, housing, food, medical care, and recreation; and provide crisis intervention. On-call Case Management is available 24 hours/7 days a week for supportive assistance whenever needs arise. Required Education/Experience: Bachelor’s degree in a human services related field (sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education) OR an Associate's Degree with 12 College Credits relevant to the position (Human Services, Psychology, Sociology) and two (2) years’ experience in the mental health/human service field. Fluent in American Sign Language. Required Licenses/Certifications: Valid driver’s license. Required Competencies: Ability to Access Community Resources: Assisting consumers in gaining access to public and private resources/services. Excellent Verbal and Written Communication Skills: Communicate with consumers on a weekly basis to determine progress and revise the service plan as necessary. Problem Solving Skills: Assist consumers with resolution of problems and/or concerns. Interpersonal Skills: Develop and enhance consumer networks of social supports. Cognizance: Observe and respond to significant changes in consumer mental and physical health. Team Player: Collaborate with county Office of Mental Health/Intellectual Disability personnel and other service providers. Flexibility: Provide 24-hour on-call crisis services on a rotational basis. Time Management Skills: Maintain case records and complete appropriate documentation as needed. Following Policies and Procedures: Understand and follow company policies. Meeting Basic Work Expectations: Comply with basic job requirements and tasks. Essential Job-Related Responsibilities: Service Plan Development: Develop service plans, treatment plans, and individual crisis plans based on the client’s strengths and needs. Adherence to Charting Requirements: Prepares and completes all necessary documentation within required timeframes. Benefit Acquisition and Maintenance: Assists the client in gaining funding authorization for needed services. Participates in Agency Investigations. Performs Other Related Duties as Assigned. Supervisory Responsibilities: N/A – this position does not involve supervisory responsibilities. Pre-employment Screening: Complete criminal background check. Name checked in registries.
