Facility Location: Grosse Pointe Woods- 21401 Mack Ave. Grosse Pointe Woods, MI 48236 Schedule: Full Time, Monday-Friday General Summary: Under the direction of the Director of Therapy, assumes delegated responsibility and accountability for the provision of rehabilitation services according to professional standards of practice in compliance with state and federal laws and regulations. Your Responsibilities: Provides leadership and direction to interdisciplinary team for patient discharge planning, following Medicare guidelines. Reviews clinical documentation (including MDS Assessments) in relation to Medicare guidelines, discharge planning and insurance requirements. Provides leadership and problem solves issues related to the provision of rehabilitation services. Provides direct patient care, in area of professional expertise/discipline meeting the clinical and State of Michigan regulatory requirements and as outlined in the discipline specific job description. Supervises the rehabilitation department in the absence of the Director of Therapy. Strong written and communication skills to develop reports, correspondence and analysis. Strong verbal communication skills to educate, influence and support rehabilitation staff and other disciplines. Strong leadership skills. Strong customer relations skills. Working knowledge of budgeting and financial reporting. Ability to exercise mature judgment and initiative in analyzing problems and recommending solutions to provide effective rehabilitation services. Ability to understand and react effectively to the needs of the various clinical age populations significant to everyday practice, evidenced through academic/clinical education and training and/or experience. Required Qualifications: Successful completion of an accredited Physical Therapist Assistant or Occupational Therapist Assistant program.
